Scheduling

The Doctors and Staff at the Central Alberta Maxillofacial Centre are focused on providing you with the utmost care in a safe environment.

Your initial appointment will consist of a consultation regarding the specific referral request made by your dentist/medical doctor. If however, you are visiting us as a self-referral, we are pleased to provide you with a consultation focused on your specific needs. You will be provided with a diagnosis and treatment options based on the specific treatment required for you.

You will then be provided with a surgical quote, instructions for your surgery and we will do our best to schedule a surgical date that will work best with your schedule.

You will then be provided with a surgical quote, instructions for your surgery and we will do our best to schedule a surgical date that will work best with your schedule.

We would ask that you bring the following information with you to your consultation:

  • Your referral and recent panoramic (PAN) x-ray if applicable (unless forwarded by your dentist).
  • A list of medications you are presently taking.
  • If you have dental insurance, please bring the insurance card(s).
  • Alberta Health Care Card

IMPORTANT: a parent or guardian must accompany ALL patients under 18 years of age.

We require a recent PAN x-ray (within one year) from your dentist, if they do not have one or it is not sufficient for the surgeons for the required surgery then we will need to take one on the day of consultation.

We do consultations on Monday, Tuesday, Thursday and Fridays with surgeries on Wednesdays.

Our office has a direct billing option for surgery where we would require the dental pre-determination at least 3 business days (Friday) before your scheduled surgery so the calculations can be done up where you will then be contacted with the amounts owing on the day of surgery. Pre-determinations are valid with our office for 90 days after the date processed.

Most insurance companies only send the pre-determination directly to the plan member and do not send us a copy. If this is the option you desire then YOU are responsible for forwarding the pre-determination to our office. Should we not have the pre-determination forwarded to our office by the Friday before the set surgical date then we will not be able to do this option and the full amount of the surgery will be due upon arrival for the surgery.

**We require 5 business day's notice (Wednesday prior) for any cancellations regardless if the pre-determination has been received or not**
